Plains Memorial Home Health
Our Home Health agency provides needed health care and supportive services intermittently to a sick or disabled person at home. It is an alternative to hospital or nursing home care. Services are only provided under the supervision of a physician.
Skilled Nursing Care:
 Skilled nurses perform assessments, observations and evaluations.
Give injections, venipuncture, and specimen collection, wound care, cast care, ostomy care, catheter insertion and care, monitor vital signs.
IV Therapy and eternal therapy.
Special diet counseling. *Diabetic teaching
Assistance with securing appropriate supplies and equipment for home care.
Teach you and/or your family measures to care for the patient at home.
Home Health Aide:
Personal hygiene with bathing assistance, care of mouth, skin and hair.
Assist with ambulation, dressing, eating and exercising.
Supervised by a registered nurse
Home Health care is not constant care. It is intermittent care provided through scheduled visits to your home by our staff.
Hospice Care
Our Hospice provides many benefits not possible in a traditional health care setting and focuses on the unique needs of patients and their families.

We also offer bereavement services for thirteen months after a loved one has passed away to help through all the first anniversaries, birthdays and other important events. Bereavement services are also available for the community twice a year around Easter and again during the Thanksgiving/Christmas holidays.
